Insurance ID Cards
Learn how to access your insurance ID cards and/or how to get by without them.
If You Don't Have Your Altus or EyeMed ID Card
Once you receive your enrollment confirmation email, be aware that you can use your coverage without an ID.
To do this, you need to give your provider the following:
Grad Student Employees

Altus Member ID: This is your unique 9 digit identifier that you can find on your dashboard.
Altus Group #: 3001-0002
EyeMed Member ID: your UMass student ID+0
EyeMed Group #: 9794348
Postdocs

Altus Member ID: This is your unique 9 digit identifier that you can find on your dashboard.

Getting Your Altus Dental ID Cards
You can print an ID by registering on the Altus website.
If you receive an error message that your information doesn't match, it may be because of "multiple last names" (remove any spaces from the "name" field) or because we have an old zip code on file for you. Contact us to update your zip code. Once you're registered, you can download a virtual ID card, check your claims, and more.
Getting Your EyeMed Vision ID Cards
If you would like a new ID, please register on the EyeMed website which will allow you to print an ID.
If you were enrolled last year, your old card will continue to work and EyeMed will not mail a new ID card automatically.
Grad Student Employees: Wellfleet Student Health Plan ID Cards
Wellfleet Insurance provides the Student Health Plan for Graduate Employees, distributed by the University of Massachusetts by virtue of your student status. While we don't manage your health plan, know that you can get a copy of your ID card here. Note that it may be necessary to type the letter "M" before your student ID when logging in to print your ID card.
Postdocs: Blue Cross Blue Shield Plan ID Cards
Blue Cross Blue Shield provides the health plan for Postdocs. While we don't manage your health plan, know that you can get a copy of your ID card here.
